Iowa Youth Survey 2018-19

TO: Parents/Guardians of 6th, 8th, and 11th grade students

RE: Iowa Youth Survey

Norwalk students in grades 6th, 8th, and 11th will be invited to participate in the statewide Iowa Youth Survey (IYS) this fall between Oct. 1st and Nov. 9th. The IYS is administered every two years by Iowa Department of Public Health with assistance from school districts across Iowa.

This survey is voluntary and anonymous.

No student is required to fill out the questionnaire. As a parent, you have the right to refuse to have your child participate in the survey. If you refuse your child’s participation, you will need to:

  • complete the Refusal of Consent form (at the end of the attached Parent Letter)
  • submit the completed form to your child’s building principal (or designee) on or before next Friday, September 14th.

If you agree to let your child participate in the survey, no action is needed and your child will be included automatically.
